*Registration will be on a first come, first serve basis

*Late Registration fee of $50.00 will apply to registrations between May 15 – June 15, 2012.   

*All Players registered after June 15th, 2012 will be placed on a waiting list until determined if an opening still exists.  PPMH will be in contact with a player once it is determined if they can be placed. 
*Fees: Above fees are base house league fees. PPMH Rep teams require additional rep fees upon team selection. 
*Outstanding Balance: Any Player will not be considered accepted until (a) any balance from previous seasons is settled (b) registration has been paid in full by July 1st.
*Payment: If payment has not been made by July 1st then the player will be put onto the bottom of their age group waiting list and an opportunity will be given to a player on the top of the waiting list to register.
*Family rate; fourth member or more of same family-no charge of lowest fee (family rate: contact us for special instructions before registering).
*Midget –Midget players are required to register for representative teams prior to tryouts, however registration payment can be remitted up to Sept. 15th without late fee.  After Sept. 15th a late fee of $50.00 will be applied.
*Juvenile players: contact the registrar to have your name placed on a waiting list.  All Midget aged players will get placed in MJHL before Juvenile aged players.  Should there be available space then Juvenile players will be given placement on a first come first serve basis.
*Jumpstart applicants please inform the registrar that your application is being submitted.  Submissions for Jumpstart can be made starting July 1st.
*Female players, make sure to register under UYMH (Uxbridge Youth Minor Hockey) association if you intend to play girls travel rep hockey. 
*New members to PPMH must submit a photocopy of player’s birth certificate. New members that have registered and played for any other hockey organization must also complete a residential questionnaire. Contact us to make arrangements. 
*Payment Plan - Payment by cheque or money order may be made in two installments. The first installment for ½ of the registration fee dated for April 1st, 2012 and the second installment post-dated for July 1st, 2012 and both cheques MUST accompany a copy of the registration form (invoice). Returned cheques will result in player suspension until full payment has been received and cleared by Registrar and Treasurer.
*Payment of registration fees must be submitted by July 1st a late payment fee of $50.00 will apply to payments made after July 1st 2012 unless noted.

Port Perry Minor Hockey Refund Policy

Refunds will be administered, as follows:
All refund requests must be made in writing via email to

2. Refunds will take a minimum of four (4) to six (6) weeks from acknowledged receipt of request for processing and administration.
3. All refunds are subject to a $40.00 administration fee.
4. Refunds will be administered as per the schedule set out below based on the date of receipt of email.
4.1 Before third (3rd) Saturday in September – full refund.
4.2 Before November 1st – 75% of registration fee.
4.3 Between November 1st and Nov 30th – 50% of registration fee.
4.4 After November 30th – No refund
5. All refunds will be processed monthly by cheque and mailed to the player address on file.
